where's my ID card?

For Basic Vision Coverage: If you elected during open enrollment to continue coverage under the Medical PPO, HDHP or HDHP Basic or the Global Choice Plan, your basic vision provider is VSP. You do not need an ID card to receive care, simply tell your provider that you have coverage under VSP. If you still want an ID card, login to your account on the VSP website and print one. If you elected coverage under a Chevron Medical HMO, your basic vision coverage is provided by your HMO plan; there is not a separate ID card for vision coverage.

For Vision Plus Program participants: You do not need an ID card to receive care, simply tell your provider that you have coverage under VSP. If you still want an ID card, login to your account on the VSP website and print one.

If you are enrolled in the Medical PPO Plan, the High Deductible Health Plan (HDHP), the High Deductible Health Plan Basic (HDHP Basic), then you’re also automatically enrolled in the Chevron Vision Program for basic vision coverage. If you're enrolled in a Chevron Medical HMO Plan, your HMO Plan provides basic vision coverage.

vision plus program

The Vision Plus Program is a voluntary coverage option that provides additional prescription eyewear coverage. You can choose to enroll in this plan, even if you waive Chevron medical coverage.

for expatriates

If you are enrolled in the Global Choice Plan, then you’re also automatically enrolled in the Chevron Vision Program for basic vision coverage.
