You must call the Disability Management Program via the HR Service Center at 1-888-825-5247 if you are absent from work for more than five days due to illness or injury or are off work for a reason that's covered under the Family and Medical Leave Act of 1993 (FMLA) or related state leave law.

Long Union Business Leave is granted on the basis of the collective bargaining agreement between Chevron and the union involved.

length of absence

You can request Long Union Business Leave for a continuous period of over 30 days, but generally not to exceed one year. Refer to your collective bargaining agreement for specific timing provisions of this leave.

Your pay will be suspended while you're on Long Union Business Leave.

service

If you're reinstated to active employment, or if you apply for reinstatement but are not reinstated at the end of this leave, the entire period of your leave will count as vesting and eligibility service in the Retirement Plan and the Employee Savings Investment Plan. You'll receive benefit accrual service for the Retirement Plan for the first 31 days of the leave. If you're eligible to accrue health and welfare eligibility service, the entire period while on leave will count as health and welfare eligibility service.

However, if you engage in any employment other than that for which this leave is granted, if you do not return to work on the first workday after your leave ends, or if you do not apply for reinstatement, your vesting and eligibility service and health and welfare eligibility service will end on the earliest of the following: 

  • 365 days from the date the leave began. 
  • The date Chevron determines you began work other than that for which your leave was granted. 
  • The date the leave ends.

In these instances, no time on leave will count as benefit accrual service for the Retirement Plan.

If you are a participant in a company-sponsored pension or profit sharing plan (such as one of the marine pension plans) in place of or in addition to the Retirement Plan and Employee Savings Investment Plan (ESIP), the rules governing the recognition of your service for the period of this leave for benefit accrual and vesting purposes may be different from those described here. To determine the effect of this leave of absence on such a plan, call the HR Service Center.

how to request a leave

You should contact your supervisor as soon as possible if you are considering a leave or if you need to take a leave.

You will need to complete a form to request a leave under this policy. Download Form F-15 Request for Long Leave of Absence Without Pay for Union Business, and follow the instructions on the form to complete your request.

returning to work

If you apply for reinstatement prior to the expiration of your Long Union Business Leave, your rights to reinstatement will be governed by the collective bargaining agreement between your union and Chevron.

benefits during long union business leave

If your leave is 31 days or less, your medical, dental and mental health/substance abuse coverage continues and contributions are deducted automatically. If your leave is more than 31 days, your regular coverage terminates at the end of the month in which your leave begins. However, you can elect to continue coverage for yourself and your eligible dependents under the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA). Your eligible domestic partner and his or her enrolled children may also be eligible for continuation of health plan coverage that is similar to COBRA.

If you elect to continue coverage, you must pay the entire cost, plus a 2 percent administrative fee. You will receive a package from the HR Service Center that contains information about your options, including cost. You are responsible for making timely premium payments as defined by the administrative rules of the Omnibus Health Care Plan. To avoid cancellation of coverage for a late or missed payment, please call the HR Service Center within 31 days of your leave if you don't receive information regarding the billing of your premiums.

If you cancel your medical and dental coverage, it will be reinstated effective the first day of the month after you return to work. If you return to work during the same calendar year, you will be re-enrolled automatically in the same coverage options that you had before the leave began. If you return to work in a different calendar year, you can change your medical and dental options. This assumes you return to work with Chevron on the first workday following the expiration of your leave.

Your Short-Term Disability Plan coverage and your Basic Coverage and Optional Coverage in the Long-Term Disability Plan are suspended on your last day of work. Short-Term Disability Plan coverage and Basic Coverage in the Long-Term Disability Plan are reinstated when you return to work. Optional Coverage in the Long-Term Disability Plan is reinstated effective the first day of the month after you return to work.

Your Basic Life Insurance Plan coverage is suspended on your last day of work; however, coverage continues for 31 days. If your coverage is suspended, you can convert your insurance to an individual policy within this 31-day period, as provided in the plan. Your coverage will be reinstated when you return to work.

You can continue Supplemental Life Insurance Plan coverage and Dependent Life Insurance Plan coverage for yourself and your eligible dependents. To continue coverage, you must pay the required contributions. You will receive billing information from the HR Service Center.

If you are a former Texaco employee, your Former Texaco Term Life Insurance Plan coverage is suspended during your leave; however, coverage continues for 31 days. You can convert your insurance to an individual policy, within this 31-day period, as provided in the plan. Your coverage will be reinstated when you return to work. You also can continue Former Texaco Contributory Term Life Insurance Plan coverage if you pay the required contributions. You will receive billing information from the HR Service Center.

You can continue coverage in the Voluntary Group Accident Insurance Plan during your leave if you pay the required contributions. You will receive billing information from the HR Service Center. If you stop coverage, the coverage you had before the leave will be reinstated effective the first day of the month after you return to work.
You can continue Long-Term Care Insurance Plan coverage and Group Auto and Home Insurance coverage for yourself and your eligible dependents. To continue coverage, you must pay the required contributions. You will be billed directly by MetLife. For billing information, call MetLife through the HR Service Center.

By paying a 2 percent administrative fee, you can continue participation in the Flexible Spending Account Plan – Health Care Program on an after-tax basis until the end of the year in which your leave begins. You will receive billing information from the HR Service Center.

You also can cancel participation. If you cancel participation, you must re-enroll by calling the HR Service Center to resume participation, and you cannot claim expenses incurred during the period you did not participate.

If your leave is 31 days or less, and in the Flexible Spending Account Plan – Dependent Care Program you return within the calendar year in which your leave began, your contributions are suspended and then automatically resumed when you return to work. The total amount not already deducted for the year will be taken out of your remaining paychecks for the year when you return to work.

If your leave is more than 31 days, your contributions end. To resume your contributions, you must contact the HR Service Center when you return to work. You can request reimbursement of qualified expenses incurred in the calendar year in which your leave begins, as long as there's money in your account (subject to the administrative rules of the Omnibus Health Care Plan).

Vacation will be in accordance with the collective bargaining agreement in effect at the time vacation is taken.
Your participation in the Retirement Plan continues while on leave.

Your active participation in the Employee Savings Investment Plan (ESIP) is suspended during your leave. When your active participation is suspended, contributions by both you and Chevron stop. Your accounts remain invested and continue to share in earnings, gains and losses. You can make exchanges and withdrawals and take loans while on approved leave. Depending on the type of withdrawal, you may be suspended from active participation for three months. If you have a loan outstanding and are on leave for more than 30 days, call Fidelity for information about your repayment options. You can contact Fidelity through the HR Service Center.
