WIN can help you better understand your options so you can maximize your Chevron medical benefits, programs, and other resources and choose a course of action that works for you.

If you are enrolled in one of Chevron's medical plans with Anthem, you are strongly encouraged to contact WIN as the first step on your fertility journey.
- WIN will first confirm your eligibility for their services.
- WIN will also discuss the Chevron medical benefits, programs and other resources that may be available to you.
- WIN will then provide personalized guidance and support throughout your family building journey, so you get started off on the right foot.

The Medical PPO, HDHP, HDHP Basic provide coverage for medically necessary fertility treatment under the plans' Family Planning and Related Services provision.
WIN’s nurse case managers provide additional personalized guidance and support through every step of the family planning and fertility process, including:
- 24/7 access to WIN’s Nurse Care Managers for emotional guidance and support.
- Education and guidance about your plan’s treatment and medication coverage.
- Education on the egg or sperm freezing process.
- Expertise in understanding complex information and decisions regarding infertility causes, testing and treatment option success rates and risk.

Guidance to help increase efficient use of hormonal medications to avoid wastage and over-stimulation.
Education on your pharmacy dosing usage, storage, and medication side effects and also assistance in maximizing your infertility medication benefit. - Access to valuable fertility resources.
The Medical PPO, HDHP, HDHP Basic provide coverage for medically necessary fertility treatment, only. For elective, non-medically necessary fertility preservation egg and/or sperm freezing, active employee plan participants also have access to a special Elective Fertility Preservation feature under the Surrogacy and Elective Fertility Preservation Reimbursement Program.*
The fertility preservation feature of the Program provides an eligible employee with financial assistance for certain eligible retrieval and storage costs associated with elective fertility preservation, with limitations. WIN can guide you in selecting an in-network provider that fits your unique fertility preservation journey. WIN will also review and validate reimbursable elective fertility preservation expenses you submit under the Program.

The Surrogacy Reimbursement feature of the Surrogacy and Elective Fertility Preservation Reimbursement Program can provide financial assistance to eligible employees for reimbursable expenses associated with the expansion of a family through a valid surrogacy contract. This program is available to all eligible U.S.-payroll employees; however, eligible employees enrolled in the Medical PPO, HDHP or HDHP Basic also have access to WINFertility services for additional personalized support and guidance on your surrogacy and adoption journey, including:
- Education and advice on the adoption and surrogacy processes
- Explanation of your benefits and potential costs
- Review of current benefits accumulator
- 24/7 access to WIN’s Nurse Care Managers for emotional guidance and support
- Collection, review and authorization of surrogacy reimbursement claims under the Program
WINFertility can also direct you to one of the several surrogacy agencies in their network. Each WINFertility surrogacy agency offers a full range of services, including comprehensive screening of egg or sperm donors and surrogates. You'll also receive pre-negotiated discounts when you use one of WIN’s participating agencies.

The Adoption Reimbursement Program can provide financial assistance for eligible reimbursable adoption-related expenses. This program is available to all eligible U.S.-payroll employees; however, eligible employees enrolled in the Medical PPO, HDHP or HDHP Basic also have access to WINFertility services for additional personalized support and guidance on your adoption journey, including:
- Education and advice on the adoption processes.
- Explanation of programs, resources and potential costs.

WINFertility (WIN) is a free family building support program for participants in the Chevron Medical PPO, HDHP, and HDHP Basic. WIN can help you better understand your family-building options so you can maximize your Chevron medical benefits, programs, and other resources and choose a course of action that works for you.
