Should you die, your beneficiary should contact the HR Service Center to begin the process for claiming payable death benefits. Your beneficiary will need to provide your social security number to the HR Service Center for identification purposes.

Bereavement counseling is offered under the Chevron Mental Health and Substance Use Disorder Plan for U.S.-payroll employees and expatriates in the U.S. and the Chevron Corporation medical plans. Your dependents (spouse, domestic partner, children) who are enrolled in coverage at the time of your death will be eligible for survivor coverage under Chevron's health care plans, as long as they remain eligible as dependents.

Financial counseling is available through Ayco's SurvivorSupport® to help navigate financial decisions following a loss. Your surviving spouse/domestic partner or other beneficiary (if you don't have a spouse or domestic partner), and terminally ill employees are eligible for this benefit.

In addition, be sure you keep your benefit plan beneficiary designations up-to-date so payable benefits can be processed quickly and efficiently.
