onsite flu shot registration

for U.S. employees & contractors at select locations

For 2021, Chevron will not offer onsite flu shot clinics at our San Ramon or downtown Houston locations due to the COVID-19 pandemic. Some locations with frontline workers may facilitate their own events and will communicate about them locally. If you have been notified of an onsite flu shot event at your location, pick the scenario below that best applies to your situation for the applicable registration links and instructions: I am a contractor, I have Chevron prescription drug coverage with Express Scripts, I am enrolled in a Chevron Medical HMO Plan, I'm not enrolled in Chevron medical coverage.

Important: To attend a scheduled local onsite event, you must be an employee (or contractor, where permitted) who is regularly assigned to that work location. Dependents are not eligible to participate at this time. If there is no onsite event at your location or you cannot attend the event at your location, be sure to review our other alternatives for getting your annual flu shot.

Chevron contractors are currently eligible to participate in this local onsite event. You'll be required to pay for your flu shot at the onsite event using a personal check or by presenting your medical insurance ID card. CVS will confirm the cost of your vaccination at the time of your appointment.

Important registration instructions

  • At the Share Coverage Information screen, choose SKIP FOR NOW.
  • You're strongly encouraged to enter both an email and mobile phone contact. Your confirmation will be sent to these contacts.
  • To cancel your appointment, refer to the instructions on your confirmation email and/or text.
  • Follow all other instructions on screen to complete your registration.

If you are enrolled in the Chevron Medical PPO, HDHP, or HDHP Basic you have Chevron Prescription Drug Program coverage with Express Scripts. Present your Express Scripts ID card at the time of your appointment and your onsite flu shot will be at no cost to you.

Important registration instructions

  • At the Share Coverage Information screen, choose I have only non-Medicare insurance
  • At the Do you have medical insurance prompt, say NO. At the Do you have prescription insurance, say YES.
  • Using your Express Scripts ID card, enter the information required. The prescription insurance provider is Express Scripts.
  • For patient relationship, enter SELF.
  • You're strongly encouraged to enter both an email and mobile phone contact. Your confirmation will be sent to these contacts.
  • To cancel your appointment, refer to the instructions on your confirmation email and/or text.
  • Follow all other instructions on screen to complete your registration.

If you are enrolled in a Chevron Medical HMO Plan, you will need to follow a two-step process to register for your onsite flu shot event. Present your CVS Voucher with your pre-populated name at the time of your appointment and your onsite flu shot will be at no cost to you.

STEP ONE: Request a CVS Voucher (required)

You cannot register for an onsite flu shot until you have received your CVS voucher. Allow 3-4 business days to process your request. 
Day of event voucher requests cannot be fulfilled, so plan ahead.

  • Request CVS Voucher here. Be sure to choose the option for BRP employee enrolled in a Chevron Medical HMO Plan.
  • When you receive your CVS voucher, return to this screen and proceed to the registration system below.

STEP TWO: Register for the onsite flu shot event

  • At the Share Coverage Information screen, choose I have a CVS Pharmacy vaccine voucher (from employer)
  • Use your voucher to enter the information requested.
  • You're strongly encouraged to enter both an email and mobile phone contact. Your confirmation will be sent to these contacts.
  • To cancel your appointment, refer to the instructions on your confirmation email and/or text.
  • Follow all other instructions on screen to complete your registration.

If you are an eligible employee and have chosen to waive Chevron medical coverage, you will need to follow a two-step process to register for your onsite flu shot event. Present your CVS Voucher with your pre-populated name at the time of your appointment and your onsite flu shot will be at no cost to you.
