The resources, tools and services listed below are offered as part of the Healthy Heart program. Note that eligible spouses and domestic partners can use all program resources, including telephone coaching. Eligible child dependents age 18 years and older cannot use telephone coaching, but all other program tools and resources are available.

Voluntary Health Questionnaire

The voluntary WebMD online health questionnaire will help you identify your health risk factors through a series of basic questions and your numbers. You don't need to provide your numbers to take the health questionnaire, but you'll receive a more accurate result if you do. Using this information, your results will include a wellness score and a personal report highlighting your risk factors, goals and a suggested action plan to help you make healthy changes. The health questionnaire is completely voluntary, but taking and retaking your health questionnaire each year may be a helpful way to monitor your personal health.  By taking the health questionnaire you may even earn points toward  health rewards. Only you, WebMD, Chevron health care plans, and certain third party health care plan vendors will have access to your individual health questionnaire results.

A Personal Action Plan

After you complete the health questionnaire, you'll be presented with ideas and resources to reduce or manage your health risk factors (if any). These recommendations will be tailored to your personal situation based on the results of your health questionnaire.

A Personal Health Coach (By Phone)

After you take the health questionnaire, you'll have the option to be paired with a professional health coach who will work with you and provide support. You meet with your coach by phone, at your convenience, and you direct the conversation around issues and goals that matter to you. Discussing the results of your health questionnaire is often a good starting point for your conversation.

WebMD Health Coaches will work with you on any number of health areas that affect your total wellness – from stress management and exercise to tobacco cessation and nutrition. The topics discussed in your calls are personalized to you and are based on your current health status and your health goals. WebMD health coaches are qualified professionals who are educated with at least a bachelor's degree in a health-related field. In addition to their degrees, many have advanced training as:

WebMD Health Assistant (Online)

The WebMD Health Assistant provides an interactive, online approach to helping you set, meet and track your health goals. First, you select one or more health goals in the following areas:

  • Weight loss
  • Tobacco cessation
  • Nutrition
  • Exercise
  • Stress management
  • Emotional health

After you select your goals, the Health Assistant lets you choose from a number of simple activities that become part of your weekly plan to support those goals. These activities have been created and developed by WebMD health coaches. You can completely customize the activities that are important or most effective for you by clicking a "like" or "dislike" button. Your plan automatically updates based on your choices. And the best part is that over time the site learns more about you to suggest activities you might prefer.

WebMD Mobile Health Apps

WebMD mobile health apps, available through the Healthy Heart Program, are designed to help you form healthy new habits and make them stick.

  • Daily Victory helps you develop an exercise habit that will be here next year and five years from now by encouraging you to make just one good choice each day.
  • Weigh Today starts you down the road to managing your weight by helping you develop a key habit, weighing yourself, that has been shown to help people reach and maintain a healthy weight.
