Summary of Benefits and Coverage (SBCs) provide summary information about your health plans, such as benefits, copayments, deductibles, coinsurance and plan contact information. You may not be eligible for all the plans listed below; see the summary plan descriptions for eligibility information. SBCs are not required for all health plans; only those listed below. These SBCs are also available free of charge by calling the HR Service Center.

Summary of Benefits and Coverage (SBCs) are not the same as summary plan descriptions (SPDs); go to the SPD page to review the SPDs for your Chevron benefit plans.

These SBCs apply to U.S.-payroll employees, unless indicated otherwise, and are effective January 1, 2018. SBCs that apply to 2017 benefits are available here.
