Overview

Chevron has selected a new claims administrator for the Chevron Dental PPO Plan. Effective January 1, 2017, United Concordia (UCCI) is replaced by Delta Dental (Delta). A new claims administrator typically affects the administration of your plan — for example, the provider network, claims submission, phone numbers, ID cards and website addresses.

Dental PPO ID Cards

You don’t need an ID card for this plan, so one will not be mailed to you. Just tell your dental office that you’re covered by Delta Dental of California and provide your Name, Date of birth, Employer Name, and Enrollee ID number (or social security number). If you have enrolled dependents, tell them to provide your details, not their own.

What’s my Enrollee ID?

Your social security number can be used to identify you, but we all want to avoid sharing that number whenever possible. Your Enrollee ID is a safer choice. It’s available starting January 1, 2017, from the Delta Dental website — and you can see it on your digital ID card or under your Eligibility Information on the website. You can also call Delta Dental after January 1, 2017 to get the number.

Want an ID card anyway?

On or after January 1, 2017, you have two ways to access your ID card online and make a print out for your use:

Print one from your computer.

  • Go to the Delta Dental website and register as a new user. (See below.)
  • Login to Online Services.
  • Click on My ID card and print.

Pull it up on your smartphone.

  • Go to the Delta Dental website and register as a new user.
  • Login to Online Services.
  • Select My ID card from the main menu.

Delta Dental Website

To register on the Delta Dental website or mobile app:

  • Go to www.deltadentalins.com/chevron.
  • Click on Register Today in the Online Services section.
  • You’ll need to provide some basic information to verify your enrollment account. You will provide your social security number as you will not yet have your Enrollee ID. This is a one-time request only. You’ll get to setup your own username and password as part of the registration process.

Find a Dentist

Go to the Delta website, then Find a Dentist on the right side navigation. Choose Delta Dental PPO from the Select Network dropdown for the greatest savings. Delta Dental Premier is also considered in the network, but the savings are less.

Effective January 1, 2017, DeltaCare USA (Delta) will become the claims administrator for the Dental HMO Plan. A new claims administrator typically affects the administration of your plan — for example, the provider network, claims submission, phone numbers, ID cards and website addresses.

Dental HMO ID Cards

Prior to January 1, 2017 you should have received a confirmation from DeltaCare in the mail containing your primary care dentist name, your Enrollee ID, an ID card and a description of your benefits from DeltaCare. This package will also provide instructions about how you can change the primary care dentist auto assigned to you. Be sure you take action right away if you want to change to another DeltaCare USA network primary care dentist.

Just tell your dental office that you’re covered by DeltaCare USA and provide your Name, Date of birth, Employer Name, and Enrollee ID number (or social security number). If you have enrolled dependents, tell them to provide your details, not their own.

There are different coinsurance, copayment, deductible, and out-of-pocket maximum amounts depending on if you see a network or an out-of-network provider. See if your current provider is a part of the network. Research the provider network.

Get your preventive dental care, save money.

The concept is simple: When you spot a disease or illness early, the cost for treatment may be reduced. And of course, early detection is better for your health, too. Most people know that getting an annual physical and recommended screenings are obvious ways to detect risk for serious conditions like cancer, heart disease, diabetes or stroke. But many people aren’t aware of the connection between good oral health and these same conditions.

In one recent study, people with serious gum disease – or periodontitis – were 40 percent more likely to have a chronic condition such as diabetes or stroke.

Periodontitis causes inflammation and a deterioration of bone structure. This inflammation can also contribute to problems in the rest of the body. It is possible to have gum disease and have no warning signs. That is one reason why simple regular dental checkups and periodontal examinations are very important.

If you are enrolled in the Chevron Dental PPO Plan or a Dental HMO Plan in 2017, we expect you to take steps to protect your health and receive at least one preventive dental cleaning in 2017. If you do not participate in this preventive care measure in 2017, you will pay $120 more for your annual dental plan premium in 2018. More information and instructions will be sent later this year.

If you are enrolled in the Chevron Dental PPO Plan or a Dental HMO Plan in 2017, we expect you to take steps to protect your health and receive at least one preventive dental cleaning in 2017. This also applies to you while you're on expatriate assignment, if you are enrolled in a Chevron dental plan. We know dental exams — like many other preventive care exams — are not exactly fun. But there’s a reason they are covered by your plan; they are important to good health. Chevron’s dental plans already cover at least two preventive dental cleanings per year; that will continue in 2017. And covered preventive dental services are generally 100 percent paid by your plan, with no annual maximum, deductible or copayment when you use a network provider (a copayment may apply for some dental HMOs).

  1. Online. Go to the Benefits Connection login screen and choose Forgot Your PIN. If you've set up challenge questions, you will be able to access the website instantly after successfully answering the questions. Otherwise you'll have to use the mail option. It can take up to two weeks to have a new PIN mailed to you.
  2. By Phone. Call the HR Service Center. When prompted to enter your PIN, hold on the line until the system provides you with other options regarding your access. You can request that a new PIN be mailed to you. It can take up to two weeks to receive your PIN in the mail.
